PLEASE READ THE FOLLOWING INFORMATION THOROUGHLY!

The Dauphin County Area Agency on Aging (DCAAA) and the Visiting Nurse Association of Central Pennsylvania (VNA) will again provide the 2007 Flu Shot and Pneumonia Shot Clinics.

If you are age 65 and your primary insurance plan is Medical Assitance (Gateway-Assured, or Emera-Health Mercy) you MUST go to your Primary Physician for your Flu and Pneumonia Shot.

Medicare HMO Subscribers, exept those with Geisinger Gold, age 65 and over must receive shots throught their HMO.

All other Dauphin County residents age 60 and older are eligible. Shot fees will be paid for by Medicare, Geisinger Gold, Unison-Advantage, or Dauphin County Area Agency on Aging.

Please complete and bring the Flu Shot Registration Consent Form (see at the end of this page). Please bring ALL of yourmedical insurance cards including your Medicare card to the clinic. Extra Flu Shot Forms will be available at the Clinics or by calling 780-6191.

ANYONE WITH AN EGG ALLERGY MAY NOT RECEIVE A FLU SHOT BUT CAN RECEIVE A PNEUMONIA SHOT.

The appointment time procecures are as follows:

1. There will be no appointment letters sent.
2. Each Clinic will have designated time slots for shots.
3. The timeslots will be organized alphabetically according to last name.
4. The schedule on reverse side designates date, location, ande time slots.
5. If the time slot is not convenient, you may come to the clinic at a different time. In this case, we ask for your patience in receiving your shot.

2007 Dauphin County Senior Flu Shots will be given at these sites. Pneumonia shots will also be available. Persons older then 65 may receive another pneumonia shot if they received the vaccine more than 5 years ago and were less than 65 at the time of the 1st pneumonia shot. In addition, people at high risk should receive another pneumonia shot after 5 years.

Please complete and bring the flu shot form (see below) with you to the Clinic. Flu Shot Forms will also be available at the Clinic or can be requested by mail by calling 780-6191. All Pneumonia Shot recipients will receive and fill out their form at their clinic site.
