Instructions:

 Begin collecting the frequency of how often the primary drug of choice was used during the last 30 days.  

OR, if use results from blood/urine tests then that can be used.  If one does not use blood/urine tests,
please begin to administer this question to patients before they begin treatment and whenever you have a follow-up.  
Most providers indicated follow-ups at 60 day intervals.  Provide data for clients on the first and most recent data collection periods.  
 
Some clients will have transitioned from detox to inpatient to outpatient, or from inpatient directly to outpatient,
where their drug use during the past 30 days may be 0.  In those cases, please ask patients to estimate their drug use
during the time period just before beginning detox or inpatient treatment.  This is applicable only
if the patient started outpatient treatment within 30 days of inpatient completion.  
 
1.  Number of outpatient clients assessed at intake/beginning of treatment.
 
2.  Number of clients reporting less frequency of use of primary drug of choice in last 30 days (to be gathered at 60 day intervals from intake).
 
3.  Percentage (5)of Out-patient Clients reporting less frequent use in past 30 days – percentage is calculated by dividing #2
(number of clients reporting less frequency of use in past 30 days) by #1 (number of clients assess at intake).
 
4.  Check (X)  if blood/urine test was done to validate clients self-report
