Practitioners can enter data into the fields of the PDF version of the form and then print the form for signing and filing. A photocopy should be served on the District Attorney and the original with a stamped receipt from the District Attorney's Office should be filed with the Clerk of Courts.

By clicking the "Highlight fields" box in the upper right corner, the available fields become visible. The completed form can be printed by clicking the print button in the upper left corner of the form. The form includes an entry of appearance so a separate praecipe is not necessary. The address entered for the defendant becomes the new bail address and future notices will be sent there.

The form, including the Common Pleas docket number, must be filed at least seven days prior to arraignment.
